Yukon Healthy Living Program (YHLP) Referral Form

Yukon health care providers use this two-page form (YG 6572) to refer patients with chronic conditions to the Yukon Healthy Living Program. Services include chronic conditions nurse educators, clinical dietitians, cardiac and pulmonary rehabilitation, supervised exercise and home health monitoring. Staff assess client and service appropriateness at intake, and an exercise specialist screens anyone entering an exercise program.

Who completes it

The referring practitioner completes patient information, the primary reason for referral (such as COPD, asthma, diabetes, heart failure, CVD risk factors, arthritis or chronic pain), recent emergency visits or admissions, and the programs requested. A physician or nurse practitioner signs the standing order for spirometry and oxygen administration.

Submitting and payment

How to submit it
Send the form to the Yukon Healthy Living Program by email or fax, using the contacts printed on it, with the requested supporting documents such as cardiac procedure reports, discharge summaries or recent spirometry results.

Good to know

  • Cardiac rehabilitation referrals indicate a risk level based on ejection fraction and coronary disease, with angiogram, PCI or cardiac surgery reports, discharge summaries and stress test results if available.
  • The standing order lets program clinicians run spirometry before and after a bronchodilator and give oxygen at 1 to 2 L/min to an SpO2 of 88 to 92% in acute situations.
  • Possible exclusions from the exercise portion include unstable cardiac conditions, myocardial infarction within 4 weeks, severe pulmonary hypertension, severe aortic stenosis, recent pneumothorax and recent embolism.
  • Gestational diabetes referrals are contacted within 72 hours. Every program also accepts self-referral, and staff notify the primary care provider when a client requests clinical services.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can refer to the Yukon Healthy Living Program?

Health care providers use this form to refer patients, but every YHLP program and service also accepts self-referral. When a client requests access to clinical services, staff notify the primary care provider; no notice is sent when clients register for group education offerings.

Does the YHLP referral need a physician signature?

Only the standing order section must be signed by a physician or nurse practitioner. It authorizes program clinicians (registered nurse, dietitian or clinical exercise specialist) to do diagnostic spirometry with bronchodilator testing and to give oxygen in acute situations, unless contraindicated.

What is YHLP home health monitoring?

Staff assess suitability and give appropriate clients a connected health kit, which may include a tablet, blood pressure cuff, scale and pulse oximeter, to record vital signs and answer symptom surveys. The program is voluntary, does not replace primary care and is not an emergency response system.
