Note
- An area of soft tissue damage due to pressure or shear which is anticipated to evolve into a deep pressure ulcer but has not yet done so. Intact or non-intact skin with localized area of persistent non-blanchable deep red, maroon, purple discoloration or epidermal separation revealing a dark wound bed or haemorrhagic blistering. It may be painful and oedematous. It can be either warmer or cooler than adjacent tissue. Evolution into a deep ulcer may be rapid even with optimal treatment.
- For multiple sites of differing stages, assign only one code indicating the highest stage. From L89 Decubitus [pressure] ulcer and pressure area
- Coding direction for DAD and NACRS Effective April 1, 2025, prefix N is being retired. Therefore, as of April 1, 2025, it is no longer mandatory to apply prefix N to a code from category L89 Decubitus [pressure] ulcer and pressure area whenever pressure ulcer is documented solely by a regulated allied health professional. It will still be mandatory to assign a code from category L89 Decubitus [pressure] ulcer and pressure area, regardless of who documented the pressure ulcer and regardless of significance. This coding direction is intended for discharges that are abstracted as of April 1, 2025, for the Discharge Abstract Database (DAD) and National Ambulatory Care Reporting System (NACRS).From L89 Decubitus [pressure] ulcer and pressure area
