Virginia WIC-395 Request for Special Prescription

Virginia WIC uses the WIC-395 when a participant's diagnosed condition severely impairs ingestion, digestion, absorption or use of nutrients, or makes conventional foods inadequate, and a specialized formula is needed. The four-page file is a cover letter plus separate infant, child and woman pages. The provider completes sections A through D on the page for the participant's category, including an ICD-coded diagnosis and any food restrictions.

Who completes it

A health care provider authorized to write prescriptions under state law (MD, DO, PA or NP) completes every field in sections A to D and signs. WIC staff record approval, pending or denial details.

Submitting and payment

How to submit it
Fax the completed page to the participant's local WIC clinic or give it to the family to bring to WIC; forms missing any field are not approved and must be resubmitted.

Good to know

  • Formula amounts above the standard WIC amount are only possible for infants and children with qualifying Virginia Medicaid coverage and a qualifying diagnosis, and never for contract formulas.
  • The diagnosis must fit the formula: prematurity supports a higher-calorie formula but not a hypoallergenic one. Listing comparable products, such as NeoSure or EnfaCare, gives WIC flexibility.
  • Ready-to-feed formula cannot be issued for tolerance, convenience or preference, and non-contract brands such as Enfamil, Good Start and store brands cannot be provided.
  • Colic, eczema, spitting up, vomiting, constipation, slow or poor weight gain and picky eating are among the listed non-qualifying conditions.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What is the Virginia WIC-395 form?

It is the Virginia WIC Request for Special Prescription, which a provider uses to request specialized medical formula for a WIC participant. The file includes a cover letter and three versions, for infants, children and women, and requests must be submitted on the page for the participant's category.

Why was a Virginia WIC-395 denied?

Common reasons named in the cover letter include missing information, a non-qualifying condition such as colic, gas or picky eating, a diagnosis that doesn't fit the formula requested, or a non-contract brand like Enfamil. The letter notes that denial does not mean WIC staff question the provider's clinical judgment.

How long can a Virginia WIC special prescription last?

Each page offers a length of use from 1 to 6 months. On the infant page, the prescription cannot go past the infant's first birthday. If the participant needs more than one product, all of them must be listed on a single form.
