New Jersey WIC-11 Medical Documentation Form

New Jersey WIC requires this two-page form before it issues exempt infant formula, WIC-eligible nutritionals or foods that need medical documentation, such as whole milk for women and children over 2. The provider names the formula and an alternative, the amount, form and length of use, checks qualifying conditions on page 2, and marks which WIC foods the patient can eat. A WIC competent professional authority then approves or disapproves it.

Who completes it

The health care provider (MD, DO, APN or PA-C) completes the entire form, including page 2, and signs it. WIC staff complete the office-use box, with a second review by an MS or RD competent professional authority when required.

Submitting and payment

How to submit it
Fax or email the completed form to the WIC office, or have the patient return it to the WIC office.

Good to know

  • Updated medical documentation is required every three months, even though the form offers lengths of use of 1, 2, 3 or 6 months.
  • No authorization is needed for Enfamil Infant, Enfamil Gentlease or ProSobee; documentation for Enfamil AR is requested but not required.
  • The form places close medical oversight and instructions on the health care provider, stating that WIC state or local staff cannot assume that responsibility.
  • Page 2 defines failure to thrive as weight-for-age or length repeatedly below the 2.3rd percentile under age 2, or below the 5th percentile over age 2.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How often must the NJ WIC-11 be renewed?

The form states that updated medical documentation is required every three months. You can still mark an intended length of use of 1, 2, 3 or 6 months in section 1, but the WIC office will expect new documentation from the provider at three months for continued formula.

What conditions qualify for formula on the NJ WIC-11?

Page 2 lists qualifying conditions by category, including severe food allergies, milk and soy allergies, metabolic, gastrointestinal and malabsorption disorders, prematurity, low birth weight, failure to thrive and tube feeding. Nonspecific formula intolerance, lactose intolerance, participant preference and enhancing nutrient intake without an underlying condition do not qualify.

Can NJ WIC provide whole milk to a child over 2?

Only with medical documentation on the WIC-11. Whole milk appears under physical form for women and children over 2, and page 2 lists unintended weight loss as a qualifying condition for whole milk for children, and underweight or maternal weight loss or inadequate gain for women.
