Vermont School Meal Medical Statement (AOE)

Vermont's Agency of Education provides this two-page template for school and child nutrition programs when a child needs a meal modification that alters the USDA meal pattern. It first asks whether the child's IEP or 504 plan already contains the required information; if so, the form stops there. Otherwise the clinician lists foods to omit, how exposure affects the child, substitutions and needs such as texture, utensils or tube feeding.

Who completes it

A state-licensed health care professional completes the modification section and signs with a printed name; registered dietitians may also submit statements in school meals since July 1, 2025 and in CACFP since October 1, 2025. The parent or guardian's contact details go at the top.

Submitting and payment

How to submit it
Give the completed statement to the parent or guardian, who returns it to the school food service director.

Good to know

  • If the child's IEP or 504 plan already contains the required information, the template tells you to stop: no separate medical statement is needed.
  • Besides foods to omit and substitute, the form has fields for modified texture, a special utensil, tube feeding, tracking assistance and other accommodations.
  • The form refers to pages 14 and 15 of the USDA-FNS guidance Accommodating Children with Disabilities in the School Meals Programs for what a statement needs.
  • The Agency of Education pairs the form with a template procedure for requesting a modification and its Accommodating Children with Disabilities manual.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a registered dietitian sign Vermont's school meal medical statement?

Yes. The form notes that registered dietitians may submit medical statements in the School Meals Programs as of July 1, 2025, and in the Child and Adult Care Food Program as of October 1, 2025. Otherwise it is signed by a state-licensed healthcare professional.

Is a medical statement needed if the child has a 504 plan in Vermont?

Not if the IEP or 504 plan already contains the required information. The template's first question asks exactly that, and if the answer is yes, the form says to stop. If the plan lacks those details, complete the rest of the form.

What should be included on the Vermont medical statement?

List the foods to avoid or omit, briefly explain how exposure affects the child, and give recommended substitutions. Then note any modified texture, special utensil, tube feeding, tracking assistance or other accommodations, and sign with your printed name.
