Colorado Medical Statement for Meal Modification

Colorado schools and agencies in USDA child nutrition programs use this statement to accommodate a child's documented disability or medical need at no extra charge. The prescriber explains the need, the major life activity affected, the diet type, texture and liquid levels, equipment, and foods to omit and substitute, and the school implements that order.

Who completes it

A parent or school contact fills in Part A. A licensed physician (MD or DO), advanced practice nurse with prescriptive authority, PA or registered dietitian completes and signs Part B, and the parent signs a permission letting staff discuss and clarify the diet order.

Submitting and payment

How to submit it
Give the completed form to the parent to return to the school or site. CDE asks schools to post the form online and keep it available in the school office throughout the year.

Good to know

  • All Part B sections must be completed. Food textures run from IDDSI 7 easy chew to pureed, and liquids from IDDSI 3 moderately thick to 1 slightly thick, or no liquids.
  • Modifications continue until a Discontinuation Form, available from the school, asks to change or stop them. CDE strongly recommends a new diet order on a new form each year.
  • For a life-threatening food allergy with anaphylaxis, school nursing staff also complete an Allergy and Anaphylaxis Action Plan alongside this statement.
  • Under USDA memo SP 26-2017, a separate statement isn't needed if the child's IEP or 504 Plan already contains the same required information.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can sign Colorado's school Medical Statement for Meal Modification?

The form accepts a licensed physician (MD or DO), an advanced practice nurse with prescriptive authority, a physician assistant or a registered dietitian. The signer adds title, printed name, phone and date, and the parent's permission lets that professional clarify the diet order if school staff ask.

Does a Colorado meal modification statement expire?

Not automatically. The form says meal modifications continue until a request to change or stop them is made on the Discontinuation Form, which the school or site provides. CDE strongly recommends that the prescribed diet order be updated every year with a new form.

Is the Colorado form available in Spanish?

Yes. CDE posts English and Spanish versions of the Medical Statement for Meal Modification, along with a Dietary Preference Form for non-medical requests, a Discontinuation Form, a letter to parents and a Fluid Milk Disability Parent Request Form.
