Louisiana WIC Medical Necessity Breast Pump Request

This one-page request serves as a letter of medical necessity so a breastfeeding mother can get an appropriate pump through Louisiana WIC. The clinician records mother and baby details, insurance and pump coverage, the pump type and the reasons, from a NICU stay over 72 hours to low milk supply or a return to work, and sets how long a loaned pump is needed.

Who completes it

An MD, NP, PA, IBCLC or RN completes the mother's and baby's details, the pump type and the reasons for need, then signs the authorization.

Submitting and payment

How to submit it
Email the completed request to the Louisiana WIC breastfeeding referral address printed on the form for processing. A proxy may also bring it to the WIC clinic for mother and baby certification if every field is complete.

Good to know

  • Pump choices are a manual pump for short-term or occasional use, a hospital-grade electric pump with a double attachment kit, or an individual single-user electric pump.
  • Only single-user pumps are issued to mothers actively diagnosed with, or recently testing positive for, Covid-19.
  • Length of need applies only to the loaned hospital-grade electric pump, entered as a number of months or as indefinite.
  • The form asks for the mother's primary insurer and whether it covers a pump, plus WIC status and family ID, gestational age, birth weight and any formula supplementation.
