Illinois ISBE Medical Authority Modified Meal Request

Illinois schools, child care centers and summer meal sites can use this ISBE sample form to request a meal modification for a child whose physical or mental impairment restricts the diet. The medical authority describes the impairment, lists foods or ingredients to avoid with alternatives, and adds any other services needed. It covers school nutrition programs, CACFP and SFSP.

Who completes it

A state licensed healthcare professional authorized to prescribe under Illinois law, or a registered dietitian, completes section 2 and signs. The parent or guardian completes the child details and signs section 3.

Submitting and payment

How to submit it
Return the form to the person the school or sponsor names in the 'Send completed forms to' box on page 2. The sponsor records the date received and any follow-up communication.

Good to know

  • A medical statement is needed for reimbursement when a disability modification falls outside the meal pattern. Changes within the pattern don't require one under USDA rules, though operators may still ask.
  • A non-conforming meal with a milk substitute stays reimbursable only when the substitute is specified on a signed medical statement for a child with a disability.
  • Preference requests use the separate Preference Modified Meal Request Form, completed by the parent, and those meals must still meet the meal pattern.
  • Sponsors attach documentation of any follow-up communication with the medical authority to the form. ISBE has marked its special dietary needs page as archived.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

What must an Illinois medical statement for meal modification include?

ISBE lists three elements: enough information about the child's impairment for the operator to understand how it restricts the diet, an explanation of what must be done to accommodate it, and, if applicable, the foods to omit and recommended alternatives. Section 2 of the sample form asks for exactly those.

Is the ISBE meal request form available in Spanish?

Yes. ISBE posts a Spanish version of the Medical Authority Modified Meal Request Form, and of the parent's Preference Modified Meal Request Form, on its School Nutrition Special Dietary Needs page, which serves both school and CACFP operators.
