South Dakota Meal Modification Request (DOE CANS)

South Dakota's Child and Adult Nutrition Services uses one request for meal modifications in schools, child care centers and homes, and adult day care. When Part B box A is checked, the participant has a condition that limits a major life activity and the program operator wants a medical statement, so the clinician completes Part C (how the diet is restricted, foods to avoid, substitutions, texture and liquid consistency) and signs Part D.

Who completes it

The parent, guardian or participant completes Part A and, with the program operator, Part B. A licensed MD, DO, PA, NP, CNP, CNM or registered dietitian completes Part C and signs items 16 to 18 when box A is checked; the parent signs items 19 to 21.

Submitting and payment

How to submit it
The family returns the completed request to the school or agency, and the instructions recommend keeping a copy of the form that was turned in.

Good to know

  • Medical statements don't need regular renewal; a modification continues until a parent or recognized medical authority signs a statement saying it is no longer needed.
  • South Dakota accepts M.D. or D.O. signatures only from physicians licensed under SDCL 36-4-9, which excludes chiropractors, dentists, opticians and physical therapists who use the title Dr.
  • A milk substitute that doesn't match the nutrient profile of cow's milk requires a condition limiting a major life activity and a recognized medical authority's signature.
  • A parent who is an MD, DO, PA, CNP, CNM or registered dietitian may sign their own child's meal modification request.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

When does a South Dakota meal modification request need a medical signature?

When box A in Part B is checked: the participant has a condition that limits a major life activity and the program operator asks for a medical statement. A licensed MD, DO, PA, NP, CNP, CNM or registered dietitian then completes Part C and signs Part D. Box B requests met within the meal pattern need no signature.

Does a South Dakota meal modification statement expire?

No. The instructions say medical statements do not need to be updated on a regular basis, though parents are encouraged to review the request each year. The school or agency follows the modification until a parent or recognized medical authority provides a signed statement that it is no longer needed.

Is the South Dakota meal modification form available in other languages?

Yes. South Dakota's Child and Adult Nutrition Services posts translated Meal Change forms in Spanish, German and Karen alongside the English Meal Modification Request, a four-page PDF that includes the instructions and the definition of a recognized medical authority.
