Utah WIC FAFAF: Children 1 to 5 and Women

Utah WIC uses this authorization when a child 1 to 5 years old or a woman needs a medical formula or nutritional product, or changes to WIC foods and milk. The provider checks at least one diagnosis, names the product and an exact daily amount, marks foods to withhold or substitute, and sets the months of issuance. The WIC clinic records the approval and the authorization's expiration date.

Who completes it

A Utah state-licensed prescriptive authority (MD, DO, NP or APRN, PA or CNM) completes sections A through F and signs; a written or stamped signature is accepted. Section A also asks for the primary care and discharging physicians.

Submitting and payment

How to submit it
Give the completed form to the WIC participant, or fax it to their WIC clinic using the clinic fax list on the Utah WIC professionals page.

Good to know

  • Ranges are not accepted; if a specific amount per day isn't given, WIC can't provide the product. WIC's maximum is 910 ounces a month, about 30 ounces a day.
  • If the patient is on Medicaid and meets its requirements, Medicaid should be the primary provider of the needed formula or product.
  • Pureed fruit and vegetable or infant cereal substitutions require a diagnosis that calls for a medical formula or product; personal preference isn't allowed.
  • With no months marked, WIC issues six months including the current month, and the order runs through the end of the final month.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I write a range on the Utah WIC FAFAF?

No. Section C says ranges are not accepted and that the product cannot be provided if a specific amount per day isn't given. Check one of the listed ounce amounts or containers per day, or write a single exact figure under Other.

Can Utah WIC give whole milk to a child over 2?

Yes, with this form. Standard issuance for children over 2 and women is skim or 1% milk, and section D lets the provider substitute 2% or whole milk. For children 1 to 2, standard issuance is whole milk, with skim, 1% or 2% available as substitutions.

How long is a Utah WIC FAFAF valid?

The provider chooses 3, 6, 9 or 12 months, or another period under 12 months, in section E. If nothing is marked, WIC issues six months including the current month, and the order continues through the end of the month.
