Mississippi WIC Form 263 Medical Formula Request

Mississippi WIC uses this request to document the medical need for formula or a medical food for women and for children 1 year and older. The provider records current weight and height, the qualifying diagnosis, the product, amount per day, tube-feeding status and duration, and checks which WIC foods the participant can eat. The clinic reviews it and scans it into the participant's record.

Who completes it

A physician or licensed medical professional with prescriptive authority under state law completes the form, including the oral intake evaluation, and prints and signs their full name with professional designations.

Submitting and payment

How to submit it
Submit the form to the participant's WIC clinic for approval, by fax or through the family. It should be no more than 60 days old when the clinic receives it.

Good to know

  • Duration is capped at 3 months, entered as 1 to 3 months, so continuing formula needs regular renewal.
  • The daily amount can be written as the number of cans or bottles, or as total ounces or milliliters.
  • The oral intake evaluation checks the foods the participant can be offered, from milk, cheese and eggs to beans, peanut butter and tuna for breastfeeding women, or formula only.
  • Approved forms are scanned into the participant's SPIRIT record, and paper copies kept at the clinic are retained for three years.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
