Minnesota WIC Medical Formula Request (Exhibit 7-D)

Minnesota WIC requires a qualifying medical diagnosis before it provides medical formula or food to infants, contract formula to children over 1, or medical food to women. This form collects the formula, diagnosis, amount, length of use and food package changes. WIC staff review and approve requests under federal regulations and Minnesota WIC policy.

Who completes it

The prescribing provider completes sections A to E. Every field in section E is required: signature, date, printed name, credential (MD, NP, PA, CNM or DO), medical office and phone.

Submitting and payment

How to submit it
Fax the form to the patient's WIC office or have the patient return it there. The Minnesota WIC Agency Directory lists each clinic.

Good to know

  • Only formulas on page 2 are allowed. Contract formulas, Enfamil Infant, Gentlease, A.R. and Reguline and Similac Isomil, need no documentation unless the child is over 1.
  • Formula intolerance, spitting up, weight management and failure to thrive without an underlying condition are not acceptable. Premature birth qualifies only under 18 months.
  • If amount or length of use isn't specified, WIC may provide up to its maximum for up to 6 months, and the maximum may not meet full needs.
  • Checking 'comparable formula' lets WIC swap within type during a recall or shortage: EnfaCare and NeoSure, or Nutramigen, Alimentum and Extensive HA.
