Texas WIC Metabolic Formula Request EF13-06-13158

Texas WIC uses this one-page request for participants with an inborn error of metabolism who need a metabolic formula or medical food. It records the qualifying diagnosis and measurements, the formula with amount per day, unit size and units per month, the issuance length, and any changes to infant or child foods. It also names the patient's metabolic center and nutritionist, and local WIC clinical staff review every request.

Who completes it

The prescribing provider (MD, DO, PA or NP) completes sections A to C, adding the metabolic nutritionist, metabolic center, contact frequency and an individual NPI, and signs or stamps it. WIC enters its local agency number.

Submitting and payment

How to submit it
Fax the completed request to the patient's local WIC clinic, or print it for the patient to bring to the clinic.

Good to know

  • Section C points to the Texas DSHS list of approved metabolic centers and asks how often the center is in contact with the family: monthly, quarterly or other.
  • Issuance can be requested for 3 months, 6 months or another length, with the amount given per day plus unit size and units per month.
  • If the food sections are left blank, the WIC dietitian or nutritionist decides the supplemental foods; you can instead request formula only, omit foods or provide baby foods for a child.
  • Therapeutic formulas that aren't metabolic use a different Texas WIC form, F13-06-13152.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Texas WIC metabolic formula request?

A health care provider marked MD, DO, PA or NP signs or stamps it, with a printed name, phone, date, individual NPI and fax. The same section names the metabolic nutritionist and metabolic center caring for the patient and how often they are in contact.

Can a child on metabolic formula still get milk from Texas WIC?

Yes, if the provider asks. The children's food section has an option to provide milk in addition to formula, plus options to issue medical formula only, omit specific foods such as eggs, cheese or peanut butter, or provide baby foods when the child can't eat table foods.

What is the difference between the Texas WIC metabolic and therapeutic formula forms?

The metabolic request, EF13-06-13158, is for metabolic formulas and foods and asks for the patient's metabolic center and nutritionist. Other medically indicated formulas go on the Texas WIC Medical Request for Therapeutic Formula, F13-06-13152. Both are posted on the Texas WIC formula prescriptions page.
