Michigan WIC DCH-1326 Special Formula/Food Request

Michigan WIC requires this two-page request before it issues a special formula, gives a contract formula to a child 12 months or older, or changes WIC foods for a client with a qualifying condition. The prescriber checks the condition, names the formula with its daily amount and duration, and can restrict or substitute foods. WIC may call the office for more information before issuing.

Who completes it

A health care provider licensed to write prescriptions under Michigan law completes all applicable sections, including the client's name, date of birth and parent or guardian, and signs Section 4 with name, address, phone and fax. WIC clinic staff complete the clinic-use box, which can record an approved-through date.

Submitting and payment

How to submit it
Send the original written request to the local WIC clinic by a secure method such as fax or encrypted email. A telephone order to a WIC Competent Professional Authority is allowed only when necessary, and the signed form must follow within two weeks.

Good to know

  • Section 1 conditions include preterm birth under 37 weeks, birth weight of 5 lbs 8 oz or less, failure to thrive, severe food allergies, and immune, metabolic or gastrointestinal disorders.
  • Rash, non-specific intolerance, underweight, fussiness, colic, vomiting, gas and constipation do not qualify, and WIC will not give formula only to boost intake or manage weight.
  • Duration cannot exceed 12 months. Checking Yes for comparable formulas lets WIC issue an equivalent from the page 2 list when the named formula is unavailable.
  • For clients 12 months and older on formula, the form can swap in infant fruits, vegetables or cereal for texture, or plant-based beverage, whole or 2% milk for low-fat milk.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What conditions qualify for a Michigan WIC special formula?

Section 1 lists preterm birth under 37 weeks, low birth weight of 5 lbs 8 oz or less, failure to thrive, severe food allergies, immune system disorders, inborn errors of metabolism, gastrointestinal or malabsorption disorders and other conditions that impair nutrition. MDHHS instructions say colic, gas, constipation, fussiness, vomiting, rash and non-specific intolerance are not indications for a special formula.

Can I phone in a WIC formula order in Michigan?

Only when necessary. The DCH-1325 instructions allow a telephone order to a WIC Competent Professional Authority if it contains all the required medical documentation, but the written DCH-1326 must reach the WIC clinic within two weeks. Otherwise send the original request by fax or encrypted email.

How long can a Michigan WIC formula request last?

The duration you enter in Section 2 cannot exceed 12 months. You name the formula from the full Authorized WIC Formulas list and the ounces needed per day; MDHHS notes that WIC maximums may not meet a client's full nutritional needs. Section 3 is optional, and if left blank WIC chooses foods from 6 months.
