DC WIC Medical Prescription Form (June 2026)

DC WIC requires this federally mandated form before it issues an exempt infant formula or WIC-eligible nutritional for a qualifying medical condition. Infants who aren't exclusively breastfed otherwise receive Similac Advance, Sensitive, Gentle Comfort or Soy Isomil. The prescriber gives the formula, daily ounces, duration and diagnosis, and a WIC CPA authorizes it and sets the expiration date.

Who completes it

A provider with prescriptive authority completes the required sections and signs, checking a credential: MD, DO, PA, CRNA, CNM, CNP, CRNP, NP or DNP. Weight, length or height and hemoglobin are optional, and the WIC CPA signs the WIC-use block.

Submitting and payment

How to submit it
Fax the form to the patient's WIC clinic or give it to the family to bring to their WIC appointment. DC WIC posts the current form and its 2026 formula listing on its WIC providers page.

Good to know

  • Fussiness, spitting up, gas, constipation, lactose intolerance, non-specific intolerance, preference, boosting nutrient intake or managing weight without a medical condition do not qualify for exempt formula.
  • Duration is 3 or 6 months, and any other length needs a written justification. WIC provides powdered exempt formula, with ready-to-feed only in limited circumstances.
  • Food requests include letting the WIC dietitian decide, formula only with no solids, or listing WIC foods not to issue. This food request section is required.
  • Listed diagnoses: GERD, severe food allergy, cow's milk protein allergy, malabsorption, failure to thrive, prematurity or low birth weight, developmental, metabolic and immune disorders, and inappropriate growth pattern.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Which credentials can sign the DC WIC Medical Prescription Form?

The form lists MD, DO, PA, CRNA, CNM, CNP, CRNP, NP and DNP as credentials for a health care provider with prescriptive authority. The provider prints their name and phone number, signs and dates the form. All requests remain subject to WIC approval by an authorizing CPA, who records the date received and the prescription expiration date.

Can DC WIC give whole milk to a child on special formula?

The provider can opt in to whole milk or 2% milk for children over 2 and for women, replacing the nonfat or low-fat milk that is standard for those groups. The same food section can request infant cereal and jarred fruits and vegetables for children over 1 and women.
