Iowa WIC 470-0092 Request for Special Formula and Food

Iowa WIC requires medical documentation to authorize special formulas and supplemental foods, and this three-page request is the state's form for it. The provider gives the formula and physical form, ounces per day, preparation, length of use and a qualifying condition with its ICD-10 code, then decides whether WIC dietitians may choose the foods.

Who completes it

An MD, DO, PA or ARNP completes sections A to E and signs. The participant or caregiver signs the section F release, and WIC staff complete section G.

Submitting and payment

How to submit it
Fax the form to the patient's local WIC clinic or give it to the family to bring to WIC. It must be filled out in its entirety to be valid, and approval is subject to USDA and WIC policy.

Good to know

  • Spitting up, milk or formula intolerance, constipation, fussiness, gas and picky eating are not accepted. Formula isn't provided to enhance intake or manage weight without an underlying condition.
  • Length of use runs 1 to 6 months. If ounces per day aren't specified, WIC may provide up to its maximum allowable, which might not meet the participant's full needs.
  • Failure to thrive must name the underlying medical condition, and allergy, immune, metabolic, gastrointestinal and other listed conditions each ask the provider to specify.
  • Section D either authorizes the WIC dietitian to choose foods or lists allowed foods, including formula only, with more formula for infants 6 to 12 months who can't eat solids.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign Iowa's Request for Special Formula and Food?

Section E calls for the signature of an MD, DO, PA or ARNP, with printed name, medical office, phone and date. The patient or caregiver separately signs the bilingual release in section F, which lets WIC and the provider share the information used to provide nutrition services.

Does Iowa WIC accept a letter or prescription instead of form 470-0092?

Iowa WIC policy accepts other written documents if they contain all the required elements. The 470-0092 lays those elements out in one place: formula, physical form, daily amount, preparation, length of use, qualifying condition with ICD-10 code, food decisions and the prescriber's signature and credentials.
