Washington F252-007-000: Hearing Impairment Worksheet

Washington L&I's Office of the Medical Director provides this one-page worksheet to calculate a worker's hearing loss for an occupational hearing loss impairment award. Using audiogram thresholds at 500, 1000, 2000 and 3000 Hz, the examiner computes each ear's monaural loss with the 1969 ANSI formula, adds up to 5% for tinnitus, then combines both ears into a binaural percentage weighted toward the better ear.

Who completes it

The attending doctor or consultant rating the hearing loss completes it from the audiogram. L&I's policy bases the degree of impairment on an audiogram interpreted by a physician.

Submitting and payment

How to submit it
Submit it to L&I with the hearing loss examination report and the audiogram it is based on.
Who pays
A comprehensive hearing loss exam with impairment rating by the attending provider or a consultant is billed as local code 1190M, with office visits bundled into the rating fee.

Good to know

  • The audiogram must be done more than 14 hours after the last noise exposure, and the worksheet records the hours since that exposure.
  • Monaural loss: average the four thresholds, subtract the 25 dB fence and multiply by 1.5; stop if an ear's four-frequency total is 100 or less.
  • Binaural loss: multiply the better ear's percentage by 5, add the worse ear's, and divide by 6; stop if either monaural loss is zero.
  • Under RCW 51.28.055, workers are not eligible for a disability payment if they do not file a claim within 2 years of the last injurious exposure.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What formula does Washington use for occupational hearing loss?

The worksheet uses the 1969 ANSI method. For each ear, average the thresholds at 500, 1000, 2000 and 3000 Hz, subtract 25 dB and multiply by 1.5, then add 0 to 5% for tinnitus. Binaural loss is five times the better ear plus the worse ear, divided by six.

How long after noise exposure should the audiogram be done?

More than 14 hours. The worksheet has a field for hours since the last exposure to noise and states it must be more than 14. It also records the date of the audiogram, since the whole calculation is based on that test's thresholds.
