WA F245-453-000 Physical Medicine Progress Report

Washington physical and occupational therapists treating injured workers file this report monthly or by the 12th visit, whichever comes first. It covers visits, cancellations and no-shows, work status, functional progress, up to four essential job tasks, objective measurements, outcome scores, barriers to recovery and the treatment plan. L&I relies on it for further authorization, and the attending provider can respond on page two.

Who completes it

The physical or occupational therapist completes sections 1-5 and signs. The attending provider may complete the optional section at the end, agreeing with the plan, ending treatment or changing it, and returns a signed copy to the clinic.

Submitting and payment

How to submit it
Fax a copy to L&I and send the form to the attending provider, ideally before the AP's next visit. If the AP signs and returns it, send that copy to L&I as well.
Who pays
The report is required documentation for physical medicine services, and L&I may deny authorization for visits beyond 24 if a progress report and chart notes are not on file.

Good to know

  • At least one completed progress report must be on file before the therapy hotline will authorize visits 13-24 on a claim.
  • Frequencies follow L&I's definitions: never, seldom (1-10%), occasional (11-33%), frequent (34-66%) and constant (67-100%), each marked against goal met, improved, no change or worse.
  • Self-reported outcome scores, such as the Oswestry Disability Index, should follow L&I's Documenting Functional Improvement resource.
  • Services the AP can consider for barriers include behavioral health, vocational help, job modification and activity coaching; a Spanish version (F245-453-999) exists.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How often is the L&I Physical Medicine Progress Report due?

Monthly or by the 12th visit, whichever comes first. L&I's physical medicine policy repeats that progress reports are due after 12 visits or every month, and warns that requests for visits beyond 24 may be denied if a progress report and chart notes are not on file.

Does the attending provider have to sign the progress report?

The AP section is optional. It lets the attending provider agree with the recommendations and update the Activity Prescription Form, state that no further treatment is needed, or change the plan of care. If the AP returns a signed copy to the clinic, the therapist sends it on to L&I.
