Vermont Cognitive Work Capabilities Form (2024)

Vermont's Department of Labor publishes this form so a treating clinician can document the work ability of a patient whose injury or illness affects cognitive function. Abilities in three categories, cognition, self-regulation and resilience, are rated, and job modifications are chosen for each. The result can release a patient to work, guide accommodations or vocational placement, and support workers' compensation, disability or SSDI claims.

Who completes it

The treating medical or psychiatric provider completes and signs it. Occupational therapists, speech-language pathologists, neuropsychologists, clinical psychologists and counselors may complete sections within their professional scope as a communication tool with the primary treating provider.

Submitting and payment

How to submit it
Give the completed form to the people who use it, such as the employer setting up accommodations, a vocational rehabilitation counselor, or the workers' compensation or disability insurer.
Who pays
The Department of Labor's form and quick guide do not set a fee for completing it or say who pays.

Good to know

  • Ratings run 1 to 4 (no limitation, needs environmental modifications, needs close supervision or assistance, unable despite supervision); 5, unable to estimate, is reserved for missing information or items outside the provider's practice.
  • If a category is rated mostly with 4s, the form asks you to consider whether job modifications make sense there, because they may not promote ability to work.
  • The final section marks an overall level, from working without modifications through modified hours or sheltered employment to no cognitive work capability, with a reassessment date.
  • List the sources behind the opinion, such as patient interview, cognitive or psychological testing, record review or employer reports of work performance, and your time on the care team.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Who can fill out Vermont's Cognitive Work Capabilities Form?

The treating medical or psychiatric provider signs it. The quick guide also lets occupational therapists, speech-language pathologists, neuropsychologists, clinical psychologists and counselors complete sections within their professional ability, as a way to communicate with the primary treating provider, whose name and signature close the form.

Can the form be used outside workers' compensation?

Yes. Vermont's quick guide lists uses beyond workers' compensation: releasing a patient to work, helping an employer put accommodations in place, helping a vocational rehabilitation counselor with placement, and documenting work disability for short or long term disability or Social Security Disability Insurance.

How should I rate a patient who lacks insight into their limits?

The guide suggests a functional interview using concrete examples from self-care, home management and job performance, such as asking whether the patient can cook several foods at the same time to gauge multitasking. It also asks raters to stay aware of unconscious biases related to age, race, education or socioeconomic status.
