Con Edison Life-Support Equipment Form

Con Edison asks households where someone uses life-support equipment, or where a resident has a serious illness, to register so it can reach them in an emergency or power outage and apply New York's HEFPA shutoff protections. A medical doctor, nurse practitioner, physician assistant or local board of health official certifies the condition and equipment. The same PDF serves as Con Edison's Certification of Medical Emergency form.

Who completes it

The customer provides account and household details. The medical professional completes Step 3, the medical verification: condition, equipment, license and signature; the fillable PDF includes e-signature fields.

Submitting and payment

How to submit it
Email or mail the registration form with the medical certification, or complete the form in My Account; the certification is due within 30 days of Con Edison receiving the customer's information.
Who pays
The form lists no fee for the medical professional's certification.

Good to know

  • Con Edison asks customers to register even when electricity is included in the rent, so it knows to contact the household during emergencies and outages.
  • A medical emergency exists when a household resident has a serious illness; it is certified by the same professionals and through the same steps as life-support registration.
  • Customers 62 or older, 18 or younger, or with a permanent disability may qualify for separate special protections and payment programs.
  • Customers can also name a relative, friend or organization to receive a notice when the bill is overdue and service might be shut off.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can certify Con Edison's Life-Support Equipment form?

A medical doctor, nurse practitioner, physician assistant, or local board of health official. The professional completes the medical verification step with the patient's condition, the equipment used, their license information and signature. The certification must reach Con Edison within 30 days of receiving the customer's information.

Is the Con Edison life-support form the same as the medical emergency form?

Yes. Con Edison links the same PDF as both the Life-Support Equipment Form and the Certification of Medical Emergency form. A medical emergency covers a resident with a serious illness, while life-support registration covers household members who use life-support equipment.
