Virginia Form SMCC: Serious Medical Condition Form

Virginia's State Corporation Commission developed Form SMCC under 20VAC5-330 so customers of regulated electric and water utilities can delay disconnection when a household member has a serious medical condition. A physician certifies the diagnosis, required treatment, prescribed equipment and expected duration. With the form on file, the utility must delay termination at least 30 days beyond the notice's expiration.

Who completes it

The customer completes account and contact details and signs; the patient, guardian or power of attorney authorizes the physician to release information to the utility and SCC. The physician (MD or DO) completes name, contacts, license number and state, diagnosis, treatment, equipment checklist, expected duration and signature.

Submitting and payment

How to submit it
Return the completed form to the customer's electric or water utility; utility representatives or the SCC may contact the physician with questions.
Who pays
Form SMCC lists no fee for the physician's certification.

Good to know

  • The equipment checklist covers mechanical ventilator, CPAP, ventricular assist device, feeding pump, nebulizer, infant apnea monitor, hospital bed, continuous oxygen, home dialysis, refrigeration, HVAC and other.
  • Without a form on file, the utility delays termination 15 days after oral or written notice; returning the form within that time triggers the 30-day delay.
  • Customers can use the delay twice in a 12-month period, and the 30-day delays may be consecutive.
  • If service was cut within the previous 14 days, the utility must restore it on receiving the form, without requiring payment to reconnect.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
