Maryland Form PSC-801: Serious Illness or Life Support

Maryland gas and electric utilities use the Public Service Commission's Form PSC-801 to delay termination when a licensed physician, certified nurse practitioner or physician assistant certifies that losing service would aggravate an existing serious illness or prevent use of life-support equipment. An optional second section lets the customer authorize release of contact information to emergency responders during weather emergencies.

Who completes it

The customer fills in the resident's name, address, relationship to the customer and account number. Section One is completed only by the licensed physician, CRNP or PA: the serious illness and/or life-support box, name, title, license number, office contacts and signature. The customer signs Section Two if they want it.

Submitting and payment

How to submit it
Return the form to the customer's utility; the copies linked by Pepco and Delmarva Power for Maryland show their own mail, fax and email details.
Who pays
PSC-801 lists no fee for the clinician's certification.

Good to know

  • Within 30 days of submitting the certificate, the customer must enter a payment agreement with the utility for unpaid and current bills to keep service.
  • Serious illness means termination would be especially dangerous to the person's health; life-support equipment is any electric or gas device essential to prevent or relieve serious illness or sustain life.
  • Section Two only allows release of contact information to government emergency responders, and it does not give priority in restoring electric service.
  • Because the PSC prescribes the form (Rev. 02172017), the same wording appears across Maryland utilities, and filing it also enrolls the household in the utility's emergency medical equipment notification program.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign Maryland Form PSC-801?

Section One must be completed only by a licensed physician, certified nurse practitioner or physician assistant. The clinician certifies that terminating electric and/or gas service would aggravate an existing serious illness, prevent the use of life-support equipment, or both, and provides title, license number, contact details and signature.

Does PSC-801 stop a Maryland utility shutoff permanently?

No. It delays termination, and the customer must enter an agreement with the utility to pay unpaid and current bills within 30 days of submitting the certificate to continue service. The customer remains responsible for the account balance.

What is Section Two of PSC-801?

An optional customer permission for the utility to give local, state or federal emergency responder agencies the customer's address, phone and account number in a weather-related emergency, so they can check on the customer. The clinician does not complete it, and it does not give restoration priority.
