DCHA ADA Form 845: Disability Verification

When a DC Housing Authority Housing Choice Voucher participant requests a reasonable accommodation, DCHA verifies the disability and the disability-related need with a physician, licensed health professional, or professional from a social service or disability agency. ADA Form 845 is that verification step in DCHA's reasonable accommodation packet. DCHA sends the provider's verification after the participant signs a release of medical information.

Who completes it

The participant completes the request and signs the authorization, ADA Form 843 (Release of Medical Information). The physician or other qualified professional completes the verification DCHA sends, confirming the disability and why the accommodation is needed.

Submitting and payment

How to submit it
The provider returns the verification to DCHA's ADA/504 office as DCHA directs; participants submit their requests by email or through DCHA's online ADA request page.
Who pays
DCHA's packet lists no fee or payment for the provider's verification.

Good to know

  • The packet also includes ADA Form 741 General Principles, Form 849 Household Verification and Form 861 Live-In Aide Information; wait-list applicants use a separate packet.
  • DCHA says applicants, residents and participants are never required to tell it that they or a household member has a disability.
  • The published packet is a nine-page scanned PDF (4/2002 edition) without fillable fields, so it is printed and completed by hand.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can verify a disability for DCHA?

A physician, a licensed health professional, or a professional from a social service or disability agency or clinic. The verifier confirms the participant's disability and explains the disability-related need for the requested accommodation, after the participant authorizes release of medical information.

Can I download ADA Form 845?

The provider verification page is not in the published packet. DCHA sends it to the provider after the participant signs the release of medical information, so the clinician completes the version DCHA sends and returns it to the ADA/504 office as instructed.
