Alaska TA 10 Health Status Report (ATAP)

Adults in the Alaska Temporary Assistance Program (ATAP) are expected to work or take part in work activities, and this two-page report tells the Division of Public Assistance what a physical or mental condition allows. The clinician states whether the condition limits work, whether full- or part-time hours are possible, how long the limitation will last and what activities suit the person.

Who completes it

A licensed health care professional completes both pages after the patient identification block and signs. The caseworker uses the answers with the TA 5 Employability Assessment and the family's TA 2 self-sufficiency plan.

Submitting and payment

How to submit it
Fax or mail the completed form to the DPA representative named at the end of the form.

Good to know

  • The report feeds two decisions: whether the adult qualifies for a work exemption and which activities go into the family self-sufficiency plan.
  • The form distinguishes full-time from part-time capacity and asks which activities are suitable, so the plan can reflect partial ability to work.
  • Expected duration of the limitation is requested alongside the work limits and suitable activities.
  • The current edition is TA 10 (06-3053), revised September 2022, a two-page fillable PDF.
