Illinois CFS 431-A Psychotropic Medication Request

Youth in Illinois DCFS care need consent from the DCFS Guardian before a psychotropic medication is started, increased or renewed. Prescribers request it on the CFS 431-A, giving diagnoses, current medications, weight and height, and for each requested drug the dose range, target symptoms and clinical rationale. The Guardian Consent Unit reviews it and returns the signed consent.

Who completes it

The prescriber, or their office or facility staff on the prescriber's behalf, completes the whole form, which records who filled it in. The DCFS Guardian Consent Unit processes the request.

Submitting and payment

How to submit it
Submit online through the DCFS Guardian Consent Portal at guardianconsent.dcfs.illinois.gov, or fax the form to the Guardian Consent Unit; the completed consent comes back by email or fax.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • All medication request fields are required for processing, and symptoms must be listed as current or controlled symptoms, not diagnoses.
  • A weight-related plan is required when the youth's BMI is below the 10th or above the 90th percentile.
  • Annual screening labs are required for every youth taking an antipsychotic or mood stabilizer; attach them or give the date they will be submitted.
  • For children under 6, the additional rationale must include alternative treatments; rationale is also requested for polypharmacy and non-first-line medications.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How do I submit a CFS 431-A?

Online through the DCFS Guardian Consent Portal, which gives a submission confirmation with a copy of the form, or by fax to the Guardian Consent Unit. The completed consent is returned by email or fax. The portal also accepts CFS 415, CFS 431 and other consent forms.

What types of requests does the CFS 431-A cover?

A new medication, a dose increase, a renewal when consent is about to expire, a medication continued for a youth new to DCFS, a one-time emergency medication for acute symptoms, and a medication or dose started without consent. Each request is marked 'not to exceed 180 days'.

Does the youth have a say in the request?

Yes. For every request the form asks whether the side effects of the requested medications were reviewed with the youth and whether the youth objects. If the youth objects, the prescriber lists the medication and explains why the youth objects.
