TCRS TR-0056: Attending Physician's Disability Report

Tennessee Consolidated Retirement System members applying for ordinary or accidental disability retirement ask their attending physician to complete this five-page report. It records primary and secondary diagnoses, body-system findings from musculoskeletal to neoplasms, prognosis, and whether the impairment has lasted or will last 12 continuous months, prevents past work, and prevents all other gainful employment.

Who completes it

The applicant completes and signs Section 1, which authorizes discussion of the disability with TCRS. The attending physician completes Section 2, filling in only the parts that apply to the patient.

Submitting and payment

How to submit it
Return the report to TCRS by mail with all office notes, hospital summaries, test results and other available medical information attached.
Who pays
The expense of furnishing the information must be paid by the applicant.

Good to know

  • If the impairment does not prevent all other gainful employment, the physician marks the work level the applicant can perform: heavy, medium, light or sedentary.
  • System sections ask for objective data such as New York Heart Association class, pulmonary function and blood gases, best corrected visual acuity, audiology, BUN and creatinine clearance, or EEG.
  • The mental disorders section asks whether the applicant can relate to and communicate with supervisors and co-workers in a work situation.
  • TCRS covers state employees, teachers and participating local government members. Companion forms include the TR0388 application, TR0408 records release and TR0169 accidental disability report.

What disability questions does TR-0056 ask at the end?
