SC PEBA LTD Attending Physician's Statement

South Carolina public employees claiming Basic or Supplemental Long Term Disability benefits need a statement from every physician treating the disability. The Standard, which administers the Basic plan and insures the Supplemental plan, uses it to judge whether the condition is disabling, so it asks for diagnoses with ICD codes, history, functional limits, treatment, prognosis and expected return to work.

Who completes it

The employee completes Part A with identifying and plan details. Each physician the employee has seen for the disability completes Part B, covering information, history, assessment, treatment, prognosis and a signed acknowledgement.

Submitting and payment

How to submit it
Mail the completed statement directly to The Standard's Employee Benefits Department in Portland, Oregon, at the address printed on the form.
Who pays
The form states that the patient is responsible for its completion without expense to The Standard.

Good to know

  • Attach lab data and special test results such as X-rays, CT scans and EKGs, plus surgical reports, discharge summaries, chart notes and narrative reports showing functional impairment.
  • Forms may be returned for unanswered questions, and incomplete forms delay the claim.
  • Basic LTD benefits begin only after a 90-day waiting period from the date of disability; Supplemental LTD has a 90-day or 180-day waiting period.
  • Part B also asks whether the patient can manage insurance benefits and which work or job site modifications could help a return to work.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does every doctor need to complete the South Carolina LTD Attending Physician's Statement?

Yes. The claim packet tells employees that if they have seen more than one physician for the disability, each physician should complete a statement. Employees can ask their employer for additional forms, and each physician mails the completed form directly to The Standard.

Is this the same form used for PEBA disability retirement?

No. This statement belongs to the State LTD insurance claim. PEBA's disability retirement under SCRS or PORS has no physician form: medical records are gathered with an authorization, form 6255, instead. An employee may pursue both, and SCRS disability benefits are counted as deductible income by the LTD plans.
