PA SERS 144: Defined Benefit Plan Medical Report

Pennsylvania state employees applying for SERS disability retirement ask each treating physician to complete this four-page report. A reviewing physician contracted by the commonwealth bases the disability recommendation entirely on these reports and attachments, without an independent exam, so the history, findings, studies, diagnoses, treatment and evaluation must address the duties in the member's position description.

Who completes it

Each treating physician completes the Initial Medical Report on pages 2 to 4 and signs; page 1 is the notice to the physician. The member supplies the employment position description for the physician to evaluate.

Submitting and payment

How to submit it
The completed report goes back to the member, who files it with SERS along with the disability retirement application.

Good to know

  • The assessment and supporting documentation must be based on examinations and tests completed within the past 60 days.
  • For psychiatric claims, include a mental status exam, a thorough case history, symptoms and severity, and any Global Assessment of Functioning (GAF) score.
  • The evaluation should list any duties the patient cannot perform and say whether the inability is temporary or permanent, with an expected date if temporary.
  • The reviewing physician may ask for more information; SERS then notifies the member and may send additional forms for the physician. Current revision 2/22/2021.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does SERS examine the member for disability retirement?

No. The reviewing physician contracted by the commonwealth does not conduct an independent medical examination. The recommendation to SERS rests exclusively on the Medical Report forms and accompanying documentation from the treating physicians.

How recent must the SERS 144 evaluation be?

The assessment and supporting documentation should be based on examinations and tests completed within the past 60 days, so the report reflects the patient's current condition rather than an older baseline.

What does the patient need to give the physician for SERS 144?

A copy of their employment position description. The physician evaluates the patient's ability to perform the duties listed there and explains in the Evaluation section which duties cannot be performed and the reasons for that opinion.
