PERAC Regional Medical Panel Certificate (MA)

When a Massachusetts public employee applies for disability retirement, a PERAC regional medical panel examines the member and answers the retirement board's legal questions on this certificate. The posted 45-page PDF, revised May 2025, holds every version: joint accidental, joint ordinary, joint heart, lung and cancer presumption certificates, and a single-physician violent act injury version. The board relies on the panel's answers and narrative.

Who completes it

Physicians appointed to the regional medical panel, up to three with their specialties listed on the cover page, complete the whole certificate and a narrative, and each signs it. A dissenting physician uses the Minority Opinion pages. The member's treating physician does not complete it.

Submitting and payment

How to submit it
The member's retirement board supplies the claim records and current job description before the examination, and the completed certificate and narrative go back to that board through PERAC. A fillable version exists only in PERAC's PROSPER application for approved medical vendors.

Good to know

  • Panel physicians should not accept documents at the examination, except medical imaging such as X-rays, MRIs and CT scans; records come from the retirement board.
  • Permanency and causality are answered only if the panel first answers yes to Question 1, incapacity to perform the essential duties in the current job description, considering risk of re-injury.
  • In accidental disability narratives, the causation opinion must be stated in terms of medical possibility, not medical certainty, and should address aggravation of pre-existing conditions and disability at last service.
  • The ordinary disability version stops after incapacity and permanency; the heart, lung and cancer versions add guides on rebutting each statutory presumption.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a treating physician complete the Regional Medical Panel Certificate?

No. Physicians on a PERAC regional medical panel complete it after examining the member. The treating doctor's opinion goes on the separate Physician's Statement Pertaining to a Member's Application for Disability Retirement, which is part of the record the retirement board provides to the panel.

How does the panel handle a heart, lung or cancer presumption?

Each presumption version includes a guide. The panel answers no to the causation question only if evidence of a uniquely predominant non-service factor obliges the conclusion that the condition is not job-related. The heart guide gives examples such as a congenital problem, endocarditis from intravenous drug use, or cocaine or alcohol cardiomyopathy, and asks panels to quantify risk where possible.

What happens if panel physicians disagree?

Each version includes Minority Opinion pages repeating the incapacity, permanency and causality questions, so a dissenting physician can record different answers. All examining physicians certify that their findings are a professional opinion reached independently and free of undue influence.
