PSRS of Missouri Attending Physician Statement

Missouri Public School Retirement System members applying for disability retirement give this three-page statement to every treating physician. It captures chief complaints, subjective symptoms, diagnoses with objective findings and test dates, lifting and positional restrictions, and the physician's opinion on whether the member is permanently incapable of earning a livelihood in any occupation, which is Missouri's legal standard.

Who completes it

Each of the member's treating physicians completes the whole statement, from present condition and diagnosis through restrictions, member incapacity and signature. The member gives a copy to each physician and is responsible for making sure records reach PSRS.

Submitting and payment

How to submit it
The physician sends the completed statement directly to PSRS with medical and clinical records for the past three years or since onset of the condition, whichever is longer.

Good to know

  • Restrictions are graded by maximum weight and by occasional, frequent or constant frequency for lifting, carrying, pushing, walking, climbing, balance, stooping and kneeling.
  • The physician certifies that all necessary medical records have been provided to PSRS/PEERS, so records should go out with the signed statement.
  • The form also asks the last date the physician saw the member and whether improvement is anticipated that would permit a return to employment.
  • Disability retirees under age 60 may later need a Medical Certification of Disability Status for recertification.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

What is the PSRS disability standard?

Missouri law requires that the disability render the member incapable of earning a livelihood in any occupation and be of a nature that warrants the assumption it will be permanent. The statement asks the physician whether, under that standard, the member is disabled from working any job, and why.

How many years of records go with the PSRS Attending Physician Statement?

Medical and clinical records for the past three years or from the onset of the medical condition, whichever is longer. The physician sends them directly to PSRS together with the completed statement.

Does every treating doctor need to complete the PSRS form?

PSRS asks members to give a copy of the statement to all of their treating physicians, and each one completes and returns it. Members are responsible for contacting their physicians to make sure all relevant medical records are forwarded to PSRS.
