INPRS TRF Physicians Statement: Classroom Disability

Indiana teachers in the Teachers' Retirement Fund who can no longer perform classroom duties can apply to INPRS for a classroom disability benefit, and this statement is the physician's part of that application. A clinician meets it when a teacher patient brings it with the rest of the packet; it then goes to INPRS with the applicant's affidavit and the superintendent's statement for a decision.

Who completes it

The member's physician completes the physician statement. The teacher completes the Affidavit of Applicant for Classroom Disability Benefit, and the school superintendent completes the Superintendent's Statement for Classroom Disability Application.

Submitting and payment

How to submit it
The completed statement goes to INPRS as part of the classroom disability application, together with the applicant's affidavit and the superintendent's statement. The form is a PDF on Indiana's state forms site, listed on INPRS's member forms page under Disability Benefits for the TRF fund.

Good to know

  • The classroom disability benefit is specific to TRF members, the teachers covered by INPRS, and centers on inability to perform classroom duties.
  • Regular PERF and TRF disability retirement works differently: it relies on a Social Security disability award rather than a physician statement on this form.
  • The 1977 Police Officers' and Firefighters' Fund disability application has no physician section; that fund uses a local board hearing and INPRS-arranged exams instead.
  • On INPRS's member forms page, filter by the TRF fund and the Disability Benefits category to find the statement and its two companion forms.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who completes the INPRS Physicians Statement for Classroom Disability Benefit?

The teacher's physician completes it to support the member's application. Two other documents travel with it: the teacher's Affidavit of Applicant for Classroom Disability Benefit and the Superintendent's Statement for Classroom Disability Application, completed by the school superintendent.

Is the classroom disability benefit the same as INPRS disability retirement?

No. The classroom disability benefit is a TRF benefit for teachers who cannot perform classroom duties, and it uses this physician statement. Regular PERF and TRF disability retirement instead relies on the member having a Social Security disability award, so no INPRS physician statement is involved there.
