NJ PF-1061: PFRS Examining Physician Report

New Jersey police and fire employers use this one-page report for the physical examination of a new officer or firefighter enrolling in the Police and Firemen's Retirement System. A physician representing the employing agency records history and exam findings, then certifies whether the applicant is physically capable of sustaining the labors and exposures of the job. It is an enrollment record, not a disability form.

Who completes it

A physician representing the employing agency completes the entire report and signs it; there is no section for the applicant to complete.

Submitting and payment

How to submit it
The report is not sent to the Division: it must be completed within one year of submission of the PFRS Enrollment Application and kept in the employer's file for auditing review.

Good to know

  • History covers operations, diseases, nervous disorders and disability awards, followed by height, weight, blood pressure, pulse, vision, hearing, color test, teeth, mouth, nose and throat.
  • The exam section also lists chest, lungs, heart, hernia, hemorrhoids, extremities, feet and toes, reflexes, and urine specific gravity, reaction and sugar.
  • The physician checks one box: the applicant is, or is not, physically capable of sustaining the labors and exposures in the performance of their duties.
  • PF-1061 is listed with the Division's employer enrollment forms; the current revision is PF-1061-0322.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When must the NJ PF-1061 exam be completed?

The report must be completed within one year of the submission of the PFRS Enrollment Application, by a physician representing the employing agency. The employer then keeps it on file for auditing review rather than sending it to the Division of Pensions and Benefits.

Is PF-1061 used for PFRS disability retirement?

No. PF-1061 documents the pre-enrollment physical of a new police officer or firefighter. Disability retirement uses different forms, such as BD-1177, the Medical Examination by Personal or Treating Physician for PFRSNJ disability retirements.
