Minnesota TRA Medical Examination Report: Disability

Minnesota teachers applying to TRA for a total and permanent disability benefit must submit two Medical Examination Reports covering the same medical condition. The report is not posted publicly: it comes with the member's online application in myTRA or in the paper packet TRA mails on request. TRA uses the reports to decide whether the member cannot engage in any substantial gainful activity.

Who completes it

The first report must come from a licensed physician. The second may come from a physician, advanced practice registered nurse, chiropractor or physician assistant, or from a psychologist when the impairment is mental.

Submitting and payment

How to submit it
The member is responsible for getting both reports to TRA along with the Disability Benefit Application, Release of Medical Information and the employer's certification. The application must be filed within 18 months after employment ends.

Good to know

  • TRA defines total and permanent disability as inability to engage in any substantial gainful activity because of an impairment expected to last at least one year.
  • Eligibility requires three years of vesting service, two earned after the member most recently became active, and the employer must certify the impairment is the primary reason for leaving.
  • Benefits cannot be backdated more than six months before TRA receives the disability application, and a denial can be appealed within 60 days of the notice.
  • TRA requires regular medical exams after approval, typically every year for the first five years and every three years after that.

Minnesota Teachers Retirement Association (TRA) takes this form through its online services or your EMR, so there is no PDF to fill. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How many Medical Examination Reports does Minnesota TRA require?

Two separate reports covering the same medical condition. The first must come from a licensed physician; the second can come from a physician, APRN, chiropractor or physician assistant, or from a psychologist for mental impairments.

Where does a patient get the TRA Medical Examination Report?

TRA does not post it as a public download. Members sign in to myTRA and follow the online disability application steps, or call TRA to request a paper disability estimate and application packet by mail. The member then gives the report to each examining provider.

Is there a deadline for a Minnesota TRA disability application?

Yes. The Disability Benefit Application must be submitted within 18 months after employment ends because of the condition. Benefits also cannot be backdated more than six months before TRA receives the application.
