Kentucky TRS Form D-3: Physician's Disability Report

Kentucky teachers applying for a disability retirement annuity, and disability retirees under review, need their physician to complete this two-page evaluation. It records the history, a physical exam done within the last three months, diagnosis, how the condition affects job duties, treatment, and the physician's conclusion on disability, and TRS uses it to apply the eligibility test that fits the member's situation.

Who completes it

The member's physician completes the whole form, items 1 through 9 and the Physician's Conclusion, and attaches reports from examinations related to the disabling condition. The member applies separately for the disability retirement annuity.

Submitting and payment

How to submit it
Return the completed form to TRS Kentucky by mail with the attached exam reports and statements. Incomplete forms are returned, which delays the member's eligibility review.

Good to know

  • Questions 2 and 3 reflect two legal tests: a single duty-related traumatic event leaving the member unable to work in any occupation, or a disability preventing job duties for at least 12 months.
  • The examination date must fall within the last three months, and the form asks for age, weight, height, blood pressure, general condition and laboratory results.
  • Items 8 and 9 ask whether corrective measures could return the member to work and whether a rehabilitation program would help, each with an explanation.
  • The same D-3 is used for continuing reviews of disability retirees; the current version is marked Revised September 2022.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How recent must the exam be for Kentucky TRS Form D-3?

The examination date entered at the top must be within the last three months. The physician should attach reports, statements and other information from examinations performed in relation to the disabling physical or mental condition, since TRS returns incomplete forms.

What does the physician conclude on Form D-3?

After the diagnosis, functional impact, treatment and rehabilitation questions, the physician certifies whether the member is physically or mentally disabled or not disabled, then signs and dates the form and adds a printed name, type of specialist if applicable, and practice contact details.

Is Form D-3 used for KERS or CERS members?

No. Form D-3 belongs to the Teachers' Retirement System of the State of Kentucky. The Kentucky Public Pensions Authority, which runs KERS, CERS and SPRS, has no treating-physician form; its members submit records with Form 8001 for review by KPPA medical examiners.
