Maryland SRA Form 20: Physician's Medical Report

Maryland State Retirement and Pension System members open a disability retirement claim with Form 20, the Statement of Disability, and Section Three is the treating physician's medical report. The Agency's Medical Board decides the claim without examining the applicant, so the physician's history, findings, studies, diagnosis, treatment, evaluation and prognosis carry the case for ordinary, accidental or special disability.

Who completes it

The member completes Section One and signs Part One of Section Three, the authorization, and the retirement coordinator or employer completes Section Two. The treating physician completes Part Two of Section Three, the Physician's Medical Report.

Submitting and payment

How to submit it
The physician forwards the report directly to the Medical Board of the Maryland State Retirement Agency. Sections Two and Three must arrive within 45 days of the claim's submission, or the claim file is closed and the claim terminated.
Who pays
The member pays any costs of obtaining the medical records needed to support the claim.

Good to know

  • Write diagnoses in full: the report says not to use abbreviations, and any ICD or procedure codes listed need a brief description of the health problem or procedure.
  • Item VIII asks whether the applicant is permanently and totally incapacitated for the normal duties of their position, answered yes, no or undetermined, with the reason.
  • The instructions list useful attachments, such as operative and discharge summaries, EKG tracings, imaging, and six months of blood pressure readings for hypertension cases.
  • The member agrees to see Agency-designated physicians if the Medical Board needs another opinion. The current Form 20 revision is 7/19.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What is the deadline for the Maryland Form 20 physician's report?

Sections Two and Three must be completed and submitted within 45 days of the date the member's claim is submitted. If the Physician's Medical Report is not received within 45 days, the claim file is closed and the disability claim is terminated.

Does the Maryland Medical Board examine the applicant?

Not routinely. The instructions say the Medical Board decides the claim without a personal examination, so the physician's documentation must show the severity and duration of the condition. The member does agree to see Agency-designated physicians if the Board needs an additional opinion.

Who completes the other sections of Form 20?

The member completes Section One, including the claim type and any work accident, and signs the release in Section Three, Part One. The retirement coordinator completes Section Two and sends the job description, two years of evaluations and attendance records. Section Four lists points the member should know.
