Indiana State Form 56459: Confidential STD Report

State Form 56459 is the Indiana Department of Health's confidential case report for sexually transmitted diseases such as chlamydia, gonorrhea and syphilis. A clinician meets it after diagnosing or treating one of these infections. IDOH's Division of HIV/STD/Viral Hepatitis posts it alongside online versions of the same report for each of the three main infections.

Who completes it

The health care provider who diagnoses or treats the STD completes the whole report.

Good to know

  • IDOH offers a separate online REDCap report for each of chlamydia, gonorrhea and syphilis as alternatives to the paper State Form 56459.
  • HIV and AIDS are reported on IDOH's Confidential HIV/AIDS Case Report Forms, not on the STD form.
  • Viral hepatitis B and C cases have their own reporting route through IDOH's Viral Hepatitis Surveillance page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
