South Dakota Confidential Disease Report (Online)

The South Dakota Department of Health takes provider reports of reportable communicable diseases through a secure online Confidential Disease Report at sd.gov/diseasereport. Clinicians use it when they diagnose or suspect a disease on the state's reportable list. Reports go to the Department's Infectious Disease Surveillance office in Pierre, and no downloadable paper form is posted.

Who completes it

The health care provider who diagnoses or suspects the disease completes the whole online report; there is no section for the patient or an employer.

Submitting and payment

How to submit it
Submit online at sd.gov/diseasereport, or report by phone, fax or mail in an envelope marked 'Confidential Disease Report' to Infectious Disease Surveillance. Category I diseases are also phoned in immediately, including after hours; other reportable diseases are due within 3 days.

Good to know

  • Outbreaks are not reported on this form: the Department posts a separate Confidential Outbreak Report for clusters and outbreaks.
  • During business hours, a phone call reaches a communicable disease staff member, and the Department lists numbers for after-hours Category I calls.
  • The Department's Review Reportable Diseases page and 2024 Reportable Disease List poster show which diseases are Category I and which fall in the 3-day group.
  • Mailed or couriered reports must be in an envelope marked 'Confidential Disease Report' and addressed to the Department's Infectious Disease Surveillance office in Pierre.
