Minnesota Chlamydia and Gonorrhea Report Form

Minnesota health care providers use this confidential report form for lab-confirmed chlamydia and gonorrhea in Minnesota residents, as Minnesota Rule 4605.7040 requires. Beyond the diagnosis, it captures specimen sources, treatment date and regimen, and whether expedited partner therapy was given. Since September 2025 the same report can be submitted online through MDH's REDCap form.

Who completes it

The diagnosing provider completes the whole form, and a separate 'reported by' field covers staff who submit it for them. There is no patient section.

Submitting and payment

How to submit it
Submit online through MDH's REDCap survey, or fax the paper form to MDH without a cover sheet; mailing is optional if faxed, in an envelope marked confidential.
Who pays
MDH lists no fee or billing code for the report.

Good to know

  • Report only lab-confirmed cases; MDH states that laboratory reports do not substitute for physician case reports.
  • On paper, type or print in capital letters with black ink, mark boxes with an X, and do not affix labels to the form.
  • To report disseminated gonorrhea or concern over persistent infection, call MDH's STD program; congenital syphilis and chancroid are also reported by phone.
  • Page 2 reprints recommended treatment regimens and refers to CDC's 2021 STI Treatment Guidelines; case reports are private under the Minnesota Government Data Practices Act.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
