Arkansas Mandatory Animal Bite Reporting Form (Online)

Arkansas law requires bites from mammals to be reported immediately to the Arkansas Department of Health, so the rabies risk can be investigated and timely care given. Providers report through an online REDCap survey, now titled the Animal Encounter Intake Form, linked from ADH's public health reporting page. It takes about 10 minutes, must be finished in one sitting, and an investigator follows up.

Who completes it

A health care provider or office can report on behalf of the bitten person; the survey asks for the facility name, the reporter's contact details and the exposed person's demographics and address.

Submitting and payment

How to submit it
Complete the online survey immediately once the bite is identified. A phone number or email is required to move past the contact page, and supporting documents can be uploaded.
Who pays
There is no fee to file the report.

Good to know

  • The opening questions ask whether a person or a pet was bitten or scratched, whether the animal was wild or domestic, and the county where the incident happened.
  • Without a phone number or email for the reporter, the investigation may be delayed or stopped.
  • If the exposed person's full date of birth is unknown, an approximate age can be selected instead.
  • ADH states that it does not pay for or provide rabies post-exposure prophylaxis at any local health unit.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
