Nebraska HHS-9-DC Provider Disease Report Form

Nebraska clinicians report a reportable disease, poisoning or organism under 173 NAC 1 on this one-page fillable form (HHS-9-DC, Rev. 5/2016). It captures provider, patient, disease, lab and treatment information and goes to the DHHS Office of Epidemiology, or to the Douglas or Lancaster County health department for patients who live there.

Who completes it

The person reporting fills in the whole form and names the physician and clinic or institution. For patients under 19, the parent's name is also required.

Submitting and payment

How to submit it
Fax or mail it to the DHHS Office of Epidemiology; Douglas County and Lancaster County cases go to those county health departments. Nebraska also accepts reports by phone, secure electronic mail and automated ELR or eCR.
Who pays
DHHS lists no fee or billing code for the report.

Good to know

  • Mark status as case, suspected case or asymptomatic carrier, and check flags such as hospitalized, day care contact, food handler, outbreak, pregnancy and suspected food or waterborne illness.
  • The form has a blood level test result field in micrograms per deciliter, alongside the lab test, specimen source, result and collection date.
  • Healthcare-associated infection events use a separate DHHS online HAI form, after which an epidemiologist contacts the facility within 24 hours.
  • Laboratories use the separate Laboratory Report Form for Reportable Diseases, Poisonings, and Organisms instead of the provider form.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
