Ohio HEA 3334 Confidential Reportable Disease Form

Ohio providers report a case or suspected case of a reportable infectious disease under OAC Chapter 3701-3 on this one-page fillable form (Rev. 5/14), sent to the patient's local health department, which enters it in the Ohio Disease Reporting System (ODRS). It records the patient, sensitive occupations, provider, diagnosis, lab results and treatment status. HIV/AIDS is not reported on this form.

Who completes it

The health care provider or infection control professional who diagnoses or suspects the condition completes it; the ODRS number is for the health department. The 'Please submit to' block names the receiving local health department.

Submitting and payment

How to submit it
Fax, mail or email the form to the local health department of the patient's residence. Class A diseases are reported by phone immediately.
Who pays
ODH lists no fee or billing code for the report.

Good to know

  • Status distinguishes laboratory-confirmed from clinically diagnosed cases (list the symptoms), and the reason for the test is diagnosis, prenatal or repeat positive.
  • Treatment status is treated or untreated, with untreated split into will treat, unable to contact, refused treatment or referred elsewhere.
  • Sensitive occupation checkboxes flag food handlers, direct patient care, child care attendees or staff, and long-term care residents or staff.
  • Columbus's disease reporting page notes that the fillable form cannot be saved on most computers once it is filled in.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
