CDC Hansen's Disease (Leprosy) Case Report Form

CDC's Bacterial Special Pathogens Branch uses this form for national surveillance of Hansen's disease (leprosy). A clinician meets it after diagnosing or treating a case, usually when the state health department gathers the clinical details. It covers exposure history such as armadillo contact and every place the patient has lived, leprosy type, biopsy and skin smear results, treatment, disabilities and household contacts.

Who completes it

The diagnosing or treating clinician, or the state health department reporter, completes the whole form, with the reporter's contact details in the reporting information block. The first two pages explain each variable.

Submitting and payment

How to submit it
Send the completed form to CDC's Bacterial Special Pathogens Branch with all personal identifiers removed, by email, fax or the DCIPHER platform, usually through the state health department.
Who pays
CDC lists no fee or payment for submitting the report.

Good to know

  • Remove personal identifiers before sending, and keep identifying details out of the comments field, as CDC's instructions require.
  • Case status options are new, return after loss to follow-up, transfer in, relapse, reclassification or other, so relapsed and transferred patients are reported too.
  • List every US state and foreign country where the patient lived before diagnosis, including military service, with dates from and to.
  • Treatment records start and end dates for dapsone, rifampin, clofazimine or other agents, and complications cover sensory loss, eye problems and hand or foot deformities.

Hansen's Disease Case Report Form

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does CDC need the patient's name on the Hansen's disease case report?

No. CDC asks that the form be sent with all personal identifiers removed. The state's own local record ID links the report back to the patient, and the comments field should not contain identifying details. The reporter's name, phone number and email are included as the contact for the case.

What exposure history does the Hansen's disease form ask for?

Whether the patient ever had direct contact with armadillos, was ever treated for latent or active tuberculosis, was previously diagnosed or treated for Hansen's disease, or received post-exposure prophylaxis, including PEP given outside the US. It also asks for every place the patient lived before diagnosis and the patient's country of birth.

Which leprosy classifications are on the form?

Lepromatous (LL), borderline lepromatous (BL), dimorphous or borderline (BB), borderline tuberculoid (BT), tuberculoid (TT), indeterminate or other. The form also records biopsy results and skin smear grading from 1+ to 6+ or none found, with the date and the country or state of initial diagnosis.
