Wisconsin F-44151 Communicable Disease Case Report

Wisconsin Stat. 252.05 and Wis. Admin. Code ch. DHS 145 require reporting of communicable diseases, and F-44151 is the Division of Public Health's paper case report for providers. It covers demographics, the disease or condition, lab data and the reporting source. Most reports now arrive electronically through WEDSS, but the form remains for providers who fax or mail.

Who completes it

The health care provider or infection preventionist completes all four sections for every disease: Demographic Data, Disease or Condition Data, Lab Data and Reporting Source, which is required.

Submitting and payment

How to submit it
Fax or mail the form, with lab results if available, to the local or tribal health department where the patient lives. Category I diseases need a phone call within 24 hours of suspicion or confirmation; Category II reports are due within 72 hours; Category III conditions go directly to the state epidemiologist.

Good to know

  • Category I includes outbreaks of any acute illness, whether or not the cause or source is known.
  • Fields marked 'not needed for STD', such as the parent or guardian of a minor, can be skipped when the report is for a sexually transmitted disease.
  • Toxicologic and environmental disease reports go to the Bureau of Environmental and Occupational Health rather than the Bureau of Communicable Diseases.
  • Providers can register to report electronically through WEDSS; the form, edition 10/2025, is also posted as a Word file.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
