Hawaii DHS 1640 WEB: Vulnerable Adult Abuse Report

DHS 1640 WEB is Hawaii's online report form for suspected abuse, neglect or financial exploitation of a vulnerable adult, or serious self-neglect, sent to Adult Protective Services (APS). Mandated reporters under HRS §346-224 use it alongside a call to the statewide APS reporting line. Submitting it satisfies the written part of their reporting duty.

Who completes it

The reporter completes the whole online form, one report per alleged victim, covering the alleged victim, the alleged perpetrators and the incidents. Members of the public can use it too, but anonymous reports are made by phone.

Submitting and payment

How to submit it
As soon as possible, call the statewide APS reporting line and submit the online form, which can be filed after business hours. You log in with a verification code sent by email, and a PDF version of DHS 1640 is linked for reporters without email.
Who pays
Hawaii DHS lists no fee or payment for reporting.
