Alabama DHR-FCS-1593 Written Child Abuse/Neglect Report

Alabama mandated reporters use DHR-FCS-1593 to put in writing a report of suspected child abuse or neglect already made in person or by phone. Its six sections cover the children, others in the home, the persons allegedly responsible, the allegations, other pertinent information and the reporter. Alabama's reporting law, Code of Alabama Sections 26-14-1 through 26-14-13, requires physicians, nurses and other listed professionals to report.

Who completes it

The mandated reporter completes all six sections, signs and dates the form, and names the person, agency and date of the verbal report.

Good to know

  • Children from different families, including separate families in the same household, each need a separate 1593.
  • If the child was treated or evaluated by a doctor or hospital because of the abuse, Section IV asks for that provider's details if they are not the reporter.
  • Reporter information is confidential and released only by court order, and the law gives people who report immunity from civil or criminal liability.
  • Required reporters who fail to report face a misdemeanor penalty, fine or sentence under the law.
