Arizona Communicable Disease Report for Providers

Arizona requires health care providers to report selected communicable diseases under Arizona Administrative Code R9-6-202, and this 1-page fillable form is the paper option from ADHS. It collects patient information, the reportable condition with onset and diagnosis dates, lab results, and disease-specific details for chlamydia, gonorrhea, syphilis, congenital syphilis, HIV and tuberculosis, plus treatment and facility information.

Who completes it

The reporting provider or facility completes all three parts: patient information, reportable condition information and facility information, naming the provider if different from the reporter.

Submitting and payment

How to submit it
Report through MEDSIS, the state's online surveillance system, or fax the form to the local health agency using the county fax numbers printed on it. Deadlines range from 24 hours to 5 working days depending on the condition.
Who pays
There is no fee to file, and the form has no billing section.

Good to know

  • The STI section asks about pelvic inflammatory disease, the number of sex partners in the last 2 months, and a negative HIV test in the last 6 months.
  • For congenital syphilis, the form asks for the birth mother's name and date of birth.
  • The TB section asks about signs and symptoms, chest imaging, site of disease, the initial drug regimen, and TB infection in a child under 6.
  • The current version is printed Version 5/2025; the ADHS provider reporting page lists the reportable conditions.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Does Arizona have a separate STD or TB report form?

No. The one Communicable Disease Report for Healthcare Providers includes sections for chlamydia, gonorrhea, chancroid, syphilis, congenital syphilis, HIV/AIDS and tuberculosis, alongside the general reportable condition and lab result fields.

Where do I send the Arizona communicable disease report?

Report through MEDSIS or fax the form to the local health agency for the patient's county. The form prints a fax number for each county, with separate STI and non-STI lines in Cochise and Maricopa counties.
