West Virginia Centralized Abuse and Neglect Form

West Virginia's Bureau for Social Services takes reports of suspected child abuse or neglect through Centralized Intake, and this online form is the written route. Under WV Code 49-2-803, mandated reporters, including medical, dental and mental health professionals, must report within 24 hours of suspecting abuse or neglect. The report reaches the intake team only after the reporter verifies it by email.

Who completes it

The mandated reporter or any concerned person completes the whole online form, covering the victims and household members, the suspected abuse or neglect, protective caregivers, the perpetrator's access, family functioning and safety threats to responders.

Submitting and payment

How to submit it
Submit the online form on bss.wv.gov, then click the verification link sent by email; previously verified reporters can skip that step. If a child is in imminent danger, report by phone to Centralized Intake immediately instead.

Good to know

  • Serious physical abuse also requires an immediate report to the West Virginia State Police and any agency with jurisdiction.
  • Any adult who observes or credibly learns of child sexual abuse must report it immediately and within 24 hours to the Department and the State Police or other law enforcement.
  • Knowingly failing to report, or preventing another person from reporting, is a misdemeanor under WV Code 49-2-812 and can bring jail time, a fine or both.
  • Good-faith reporters are immune from civil and criminal liability under WV Code 49-2-810.

Frequently asked questions

Why didn't my West Virginia online abuse report go through?

The Centralized Intake team does not receive the report until the reporter clicks the verification link emailed after submission. Check for that email and complete the step; reporters who have verified before can skip it. For imminent danger, call Centralized Intake rather than relying on the online form.

How fast must West Virginia mandated reporters report child abuse?

No more than 24 hours after suspecting abuse or neglect, under WV Code 49-2-803. A reporter may delay only while removing themselves or the children from greater risk of serious harm, and must then report as soon as practical. Serious physical abuse also goes immediately to the State Police.
