California BCIA 8572 Suspected Child Abuse Report

California mandated reporters use BCIA 8572 for the written report required by the Child Abuse and Neglect Reporting Act (Penal Code section 11166). After a phone report to police, the sheriff, county probation or county welfare, the reporter sends this 2-page form to that agency within 36 hours. It covers the reporting party, agency notified, victim, involved parties and the incident narrative.

Who completes it

The mandated reporter completes sections A to E, one report per victim; if the report is phoned in, the person taking it may sign section A.

Submitting and payment

How to submit it
Report by telephone to a designated agency immediately or as soon as practically possible, then send the written form to that agency within 36 hours. Do not send a copy to the Department of Justice.
Who pays
There is no fee to file, and the form has no billing section.
