New Mexico APS Online Report Form (Adult Abuse)

New Mexico's Aging and Long-Term Services Department takes reports of suspected abuse, neglect, exploitation or self-neglect of adults 18 and older who cannot safeguard themselves through an online APS report form or its statewide intake line. A clinician who suspects a patient is being harmed can file online; APS investigates and, when necessary, provides protective services using the least restrictive means possible.

Who completes it

The reporter completes the whole online report. New Mexico law (NMSA 27-7-30) requires any person who suspects adult abuse to report, so clinicians report the same way as anyone else.

Submitting and payment

How to submit it
Submit the online 'Report Neglect / Abuse / Exploitation' form (WellSky web intake) linked from the ALTSD APS page, or call APS Statewide Intake toll-free. Participants in the DD, Mi Via or Medically Fragile waivers are reported to the Health Care Authority instead.
Who pays
ALTSD lists no fee for reporting.
