SC DPH 1129 Disease Reporting Form (2026)

South Carolina requires the attending physician or designee, and laboratories, to report conditions on the state reportable list under SC Code 44-29-10, 44-1-112 and Regulation 60-20. The 1129-ENG-DPH, formerly the DHEC 1129, is the paper route, covering demographics, illness and treatment, labs, viral hepatitis results and risk factors. The county health department or DPH Central Office enters it into SCION.

Who completes it

The attending physician or a designee completes the whole form, including the reporter, performing lab and ordering physician fields. Laboratories also report, and some conditions on the list are reportable by laboratories only.

Submitting and payment

How to submit it
Mail or phone reports to the health department office in the region where the patient lives, or enter them online in SCIONx; HIV, AIDS and STD reports go to the Surveillance, Assessment and Evaluation Section and must not be faxed. Urgent conditions are phoned in immediately or within 24 hours; others are due within 3 days.

Good to know

  • Animal bites and potential rabies exposures are reported within 24 hours on the online 1799-ENG-DPH Animal Incident Report Form; for rabies PEP, record the animal species and first-dose date.
  • Tuberculosis is reported to the regional TB office for the patient's county, and page 2 of the form lists which office covers each county.
  • All lead test results are reportable; mark whether each specimen was venous or capillary in the laboratory section.
  • As the form notes, HIPAA (45 CFR 164.512) allows disclosure of protected health information to public health authorities without the patient's consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How fast must South Carolina reportable diseases be reported?

It depends on the condition. The 2026 list printed on the form has three groups: report immediately by phone (for example anthrax, measles and meningococcal disease), report within 24 hours by phone (mumps, acute hepatitis, tuberculosis), and report within 3 days (chlamydia, gonorrhea, chronic hepatitis B and C).

Can I fax HIV or STD reports to SC DPH?

No. The form says not to fax HIV, AIDS or STD results to DPH. Submit them electronically through SCIONx, which DPH prefers, mail them to the Surveillance, Assessment and Evaluation Section, or phone the dedicated line to leave a detailed message.

Is the 1129 the same as the old DHEC 1129?

It replaces it. The form was the DHEC 1129 before South Carolina split DHEC and created the Department of Public Health in 2024. The current edition is 1129-ENG-DPH, revised 12/2025, printed as the 2026 SC Department of Public Health Disease Reporting Form.
